DATA SUBJECT’S EXPLICIT CONSENT FORM
In accordance with the “Personal Data Protection Law” No. 6698, all kinds of personal data, including those that determine or determine my identity and contact information provided by me orally/in writing and/or electronically, shall be processed by Lava Akustik, and shall be shared for commercial purposes with sponsoring companies that require sharing in accordance with the relevant legislation; I agree, declare and undertake that I agree to the processing of personal and contact data detailed below within the scope defined in the “Law on the protection of personal data” No. 6698 of personal data and contact information.

Personal data that is owned by a certain or identifiable natural person; processed partially or completely automatically or non-automatically as part of the data recording system; and related to the receipt and evaluation of any request or complaint filed against Lava Akustik.
Under the personal data protection Act No. 6698, my contact information and my data by the law by Lava Akustik collection, recording, processing, retention, as determined by our board of directors and corporate sponsors to be shared with stakeholders in advance I’d accepted that I’d permitted for commercial purposes.
